My development team just got back to me. Every single one of your crashes was occuring because for some reason, Direct3D was not initializing.
In my personal experience, this occurs because of either a corrupt Direct3D installation or a corrupt driver installation.
First, try reinstalling DirectX either from your retail media or from http://www.microsoft.com/directx.
If that doesn't work, download the latest ATI drivers and leave the installation file on your desktop. Then go to Add/Remove Programs and completely uninstall anything ATI related. Finally, reinstall the drivers after a reboot.
